There are many active special interest student groups and organizations on
campus. They provide various opportunities for students to socialize, have
fun, share interests, and learn new leadership skills while participating in
unique projects with peers and advisors. The following groups are open to all

Request to form a student club:
This is used if you are starting a club
for the first time.
Re-registration Form:
Use this form in September or January to re-register your club if
it has been active within the past year.
Event Proposal Form:
Use this form for any upcoming event you would like to have. Upon approval, the Office
of Student Development and Activities can help you with room reservations.
Conference Travel Request Form:
Use this form for faculty members and club advisors who wish to send their students to a conference or competition.
Table and Room Request Form:
Use this form to request a room or a table set up for your club.
Sign-In Sheet:
Use this sheet for all attendees to sign in at your meetings or events. A copy should
be turned into the Student Activities Office, SC112 immediately following your meetings
and/or events. Attendance helps us compile our annual statistics. It is also a factor
when determining budgets.
Off Campus Table Request Form:
Use this form if you are an off campus organization wanting to request the use of a table in the Student Center Building.
